To ensure thorough and timely service, please remember to bring the following to your first visit:

1. All insurance cards or claim forms
2. Photo ID
3. A copy of your relevant medical records (such as x-rays and lab tests)
4. A complete list of medications (prescriptions and over-the-counter medications)
5. Your referral form (if applicable). Please note that if you are insured by an HMO care plan, it is your responsibility to obtain your referral from your primary care provider.

If you are a new patient, we kindly ask that you arrive at least 15 minutes prior to your scheduled appointment time in order to process your information.

You may be asked to provide a urine sample as part of your exam, so we ask that you wait until seeing the nurse before emptying your bladder.
